Dr. Tara Liberman is the Executive Director of Hospice Care Network for Northwell Health since January 2022. She is board certified in Internal Medicine, Geriatric Medicine and Hospice and Palliative Medicine. Dr. Liberman is an Associate Professor at the Donald and Barbara Zucker School of Medicine.
She served as Associate Chief for the Division of Geriatrics and Palliative Medicine for Northwell health from January 2016- December 2021.
As co-director of the Geriatric Emergency Department Committee, she lead Northwell to be nationally recognized as being the first health system to obtain Geriatric Emergency Room accreditation by the American College of Emergency Physicians. She is Director of the Northwell Palliative Care Leadership Center (PCLC), one of 9 in the nation, which trains leaders of health systems on how to develop Palliative Programs.
She has been part of the faculty for the Northwell Advanced Illness and Aging Collaborative and created many of the Health System’s Goals of Care initiatives. She has been an advocate for patient and family goal concordant care through education, advocacy, communication, and documentation.
